The Medicines and Healthcare products Regulatory Agency (MHRA) recently made headlines with the approval of the first daily GLP-1 tablet for weight management in the UK.
From a clinical standpoint, this is a monumental step forward. Currently, the most effective medicines for weight management require self-administered injections. This daily oral tablet removes that barrier for anyone who was previously put off, offering a more accessible alternative for patients struggling with weight loss resistance.
Having dedicated over a decade to high-level physical training, coaching others along the way, and now stepping into my role as a qualified Personal Trainer at Clove, I feel there is a need to address a critical misconception. Medication is a tool - it is an adjunct to support weight loss, and shouldn’t be used as a standalone treatment.
Taking a GLP-1 medication on its own and not changing how you live your life is simply not enough for long-term sustainability, and is putting your long-term health at serious risk. For the weight loss to be long term, taking the medication must be paired with actively protecting your metabolism. The medication is highly effective at reducing your appetite, but it cannot do the physical work for you. This means moving beyond simply eating less. You need a clear plan that ensures you are eating enough protein to keep your body nourished, alongside regular strength training to protect your muscles and bones. Without these physical changes, the tablet is only a temporary fix, not a sustainable change.
The Hidden Danger of Unassisted Weight Loss
GLP-1 medications suppress appetite and slow down how quickly food leaves your stomach (gastric emptying) exceptionally well. Because you feel fuller for longer, you eat significantly less, which naturally results in weight loss over time.
Crucially, a distinction must be made between maintaining a healthy calorie deficit and simply starving yourself. The core difference lies in the outcome: weight loss versus fat loss.
When you radically lower your caloric intake, your body seeks internal fuel sources. If you are not consistently sending signals to your body that your muscles are required for active work, your body will choose to burn through your muscle tissue alongside your body fat. To keep your brain and vital organs running, it initiates a survival process called gluconeogenesis - literally breaking down your muscle tissue to convert it into sugar for energy. Sarcopenia, the loss of muscle mass, is metabolically detrimental. Your muscle are the engine that directly drives the number of calories your body burns every day. Losing 15kg via a GLP-1 tablet results in a significantly slower metabolism if a substantial portion of that weight loss comes from muscle mass rather than fat.
This is the exact reason why so many people experience rapid, extreme weight regain the moment they stop taking weight loss medications. They have essentially slowed their metabolism.
